Account Signatory Forms

To add a Signatory to your account, the following is required:

• Completed Authorised Signatory Form.  This form can be found here;
• Current Employment Letter - No older than one (1) month;
• Two (2) Valid Photo IDs (Drivers Licence, Passport, Voters Registration Card)

Please note that all signatures on the authorised signatory form must be witnessed by a:

• Credit Union Employee, or a;
• Justice of the Peace, or a;
• Notary Public.

The completed authorised signatory form and all required documentation can be emailed to CUconvenience@creditunion.ky 

When will My Authorised Signatory be Added To My Account?

All requests received will be completed within two (2) business days.
